What is utility of Oximes in Poisoning
Pralidoxime is available as PAM-A (pralidoxime and PAM (pralidoxirne iodide).

Vial of pralidoxime iodide 500 mg

Tablet pralidoxime chloride 500 mg

Indications:

1. In treatment of poisoning due to pesticides, chemicals and organo phosphorous compounds which have anticholinesterase activity, e.g. parathione, methyl- parathione, TEPP, isoflurophosphate, diazinon, Tik-20, etc.

Indications for oximes in organo phosphorous compound poisoning. 1. Fasciculations; 2. Muscle weakness; 3. Respiratory depression.

2. Control of overdose of anticholinesterase drugs used in the treatment of myasthenia gravis.

Dose

Adults: 1 gm IV once daily or as per need in cases of poison, slowly diluted, or 1-2 g by drip infusion diluted with 100 ml of 0.9% Nael.

Children: 20-4Omg/kg once a day

The drugs have hardly any side effects. Possible adverse reactions due to high dose of the drug itself are transient NM blockade, muscle weakness, blurred vision, diplopia, dizziness, headache, and tachycardia.

PAM must be administerd after an injection of atropine. Special precautions have to be taken in cases of Mysthenia Gravis and Renal insufficiency.
